  • Identifying information about the user that is responsible for the reported content.
  • A short description detailing your problem with the reported content.
  • The exact date and time you encountered the reported content on PLAD8.
All fields are required. Reports submitted with incomplete, invalid, or missing fields cannot be reviewed.
